	Interment Authorization
	The signed Interment Authorization form must be presented to the Cemetery Superintendent at the time of the Funeral along with the Burial Permit.

	Interment Order
	The information on this form may be phoned into the Cemetery office at (978) 851 - 2248 or faxed to (978) 458 - 4907. We will call you to confirm the receipt of this form. We will also call you to verify the information before the funeral.

	Memorial Application
	The Memorialist must submit this form for approval prior to any installation or work on a Monument or Flat Marker. This form is 2 pages long. 	Prepaid Arrangement Request Form
	To request a Prepaid Arrangement at St Mary Cemetery the cemetery requires that the Prepaid Arrangement Request Form be filled out and notarized. 	Burial space in a lot at St Mary Cemetery can be reserved for specific individuals. Please use this form to designate in writing the grave number(s), name(s) and relationship to the Original Owner of the Lot.
